Position Summary:

The Attending Nurse Practitioner (NP) is a primary care provider to residents and works within their legislative scope of practice as described and outlined by the College of Nurses of Ontario. The NP collaborates with the resident or substitute decision maker (SDM), and the health care team in the development, implementation and evaluation of the resident’s plan of care. The NP will also provide leadership and mentorship to the LTC nursing team to enhance their knowledge, assessment skills, and ability to care for residents in place, and leads and collaborates in research, education, and evidence-based practice initiatives to optimize resident, LTC and health system outcomes.

Assets and Qualifications:

Essential:

  • Registration in the Extended Class as a Nurse Practitioner.
  • Registration with the College of Nurses of Ontario.
  • Bachelor of Nursing from a recognized University.
  • Ability to communicate effectively with internal and external groups to accomplish client specific and organizational goals and objectives, excellent writing and communication skills, skill in critical appraisal of research studies, knowledge of health promotion, and demonstrated excellence in leadership in a team setting.
  • Proficient with computers and a variety of software.
  • Current registration with the College of Nurses of Ontario with the appropriate certification of registration and entitled to practice without restriction
    • proof of registration must be provided through the CNO website or some other method involving the CNO.

 

Preferred:

  • Three years of recent experience in a LTC or public/community health setting, a minimum of two years’ experience in a LTC nursing role.

Responsibilities:

  • Provides comprehensive primary care to residents as part of the interdisciplinary team.
  • Performs a person-centered health assessment of residents on admission, quarterly, and annually.
  • Collaborates with the resident to develop a person-centered plan of care.
  • Attends and participates in care conferences.
  • Manages the care of residents by providing pharmacological, complementary and/or counselling interventions, and performs procedures within the NP scope of practice.
  • Orders and/or performs appropriate screening and diagnostic investigations, interpreting results, and assuming responsibility for follow-up.
  • Collaborates and consults with physicians, the Director of Care, nursing staff, interdisciplinary team members and external resources regarding a resident’s plan of care.
  • Documents clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, resident responses, and clinical rationale in a timely and accurate manner.
  • Prescribes medications and treatment within their scope of practice.
  • In collaboration with the interdisciplinary team members, review and analyze data for trends, coordinate quality improvement initiatives and guide corrective actions in order to maximize service and minimize resident risk.
  • Works with the Senior Leadership Team to participate in evaluative processes i.e. Accreditation, Resident Quality Inspections and Continuous Quality Improvement.
  • Provides formal and informal teaching and coaching in the management of clinical care to interdisciplinary team members, serving as resource person, educator and role model.
  • Engages in evidence-informed practice by critically appraising and applying relevant research, and theory in providing health care services.
  • Engages in ongoing professional development, maintains clinical competence and currency with legislation affecting her/his practice.

Shift Information:

Non-Unionized Position

$60.00 to $67.50 per hour

Ability to provide effective service in French (proficient in conversational French for activities of daily living) is considered an asset.
